Nurse Practitioner (APRN) – Internal Medicine / Primary Care (Outpatient)

Location: Woodsville, NH (Rural Health Clinic, outpatient)
Schedule: Full time, Monday to Friday, no call. 32 patient hours / 8 admin hours per week. Onsite.
Compensation: $114,400 to $208,000 based on experience, plus potential sign-on and/or relocation bonus.
Experience: Minimum 3 years required. No new graduates.

About the Role

Are you an Advanced Practice Registered Nurse (APRN) who values meaningful patient relationships, team collaboration, and the ability to make a lasting impact on the health of your community? We are seeking a compassionate and motivated APRN to join our Primary Care team at a Rural Health Clinic that offers a variety of specialties to create a medical home-type environment.

This position provides full-scope primary care services to an adult patient population ranging from age 18 to 108. Working collaboratively with physicians, advanced practice providers, nursing staff, and interdisciplinary team members, the APRN will provide comprehensive assessment, diagnosis, treatment planning, disease prevention, health maintenance, and ongoing management of acute and chronic medical conditions.

The APRN manages an assigned patient panel and plays a vital role in promoting preventive care, managing chronic disease, coordinating specialty referrals, educating patients and families, and supporting positive health outcomes. This is an established, high-volume outpatient practice. Common conditions managed include hypertension, diabetes, hyperlipidemia, coronary artery disease, COPD, asthma, thyroid disorders, obesity, chronic kidney disease, gastrointestinal conditions, arthritis, musculoskeletal disorders, and other preventive health and wellness needs.

Qualifications

Required: APRN license, RN license, Basic Life Support (BLS)
