Mon-Fri full time (5×8, days)
$70-105k based on experience


The Opportunity

We’re looking for a compassionate and skilled Registered Nurse to join our Adult Medicine team. In this role, you’ll provide professional nursing care to a diverse adult patient population, working alongside a dedicated clinical team to deliver high-quality, patient-centered care. You’ll play a key role in triage, care coordination, health coaching, and patient education — all within a fast-paced, mission-driven environment.


What You’ll Do

  • Assess and triage patients in person and over the phone; coordinate sick visits and home care instructions
  • Administer medications and treatments per provider orders; monitor and document patient responses
  • Perform nurse visits, Point of Care Testing, and assist with clinical procedures
  • Educate patients and families on disease prevention, medications, and self-management goals
  • Coordinate lab/diagnostic appointments and manage follow-up on results
  • Collaborate with the care team on chronic disease management, referrals, and care gaps
  • Handle complex patient calls and route appropriately to providers
  • Communicate social determinants of health to the care team; assist with prior authorizations
  • Maintain infection control, quality control, and equipment maintenance standards
  • Support public health reporting requirements per local, state, and federal guidelines

What We’re Looking For

Required:

  • Active, valid Connecticut Registered Nurse license
  • Associate’s degree in Nursing (Bachelor’s preferred)
  • CPR certification or ability to obtain within 90 days of hire
  • At least one year of professional nursing experience preferred
  • Strong clinical judgment, especially in triage and urgent/walk-in settings
  • Proficiency with electronic health records and Microsoft Office
  • Excellent communication skills and ability to work with patients of all ages
  • Ability to provide culturally competent care in a collaborative, team-based environment
  • Commitment to confidentiality and HIPAA compliance

Preferred:

  • Experience with socioeconomically diverse, multiethnic populations
  • Bilingual — written and oral proficiency in Spanish or another language
