Clinic hiring for part time RN to coordinate care in HIV/homeless care department

Responsibiliites:
- onducts an intake on all new patients which includes assessing the patients understanding of HIV, past medical history and ART history; implements a culturally competent plan of care that will help the client achieve durable viral suppression and evaluates the plan at least yearly.
- Provides initial and ongoing education on HIV transmission, prevention, risk reduction, disease progression and treatment strategies; counseling on reducing barriers and adherence support.
- Provides education, counseling and adherence support through regular contact with each patient through telephone calls, office visits, agency visits, home visits, and hospital visits, according to protocol.
- Coordinates and/or provides outreach efforts to new patients to assist them engage in HIV care.
- Responsible for state mandated reporting.
- Provides nursing assessment to all patients, include triaging needs of patients waiting for clinician.
- Clearly documents data in the medical record.
- Recognizes, assesses and takes immediate action in acute and crisis situations, both patient and environmental. Able to handle emergencies, assists clinicians in stabilization of patient, and follow agency clinical emergency response procedure. Demonstrates ability to think clearly and arrive at sound conclusions. Demonstrates a clear understanding of importance of advocating for the patient and family.
Requirements:
- Minimum of 2 years’ experience in case management or care coordination preferred.
- Certified in CPR.
- Thorough knowledge of HIV infection in adults.
- Knowledge of clinical and cultural issues involved in the care of Latino, African American and gay, lesbian and transgender as well as those with substance abuse.
