The Project Access-New Haven (PA-NH) patient navigator reports to the RN navigation manager and works closely with PA-NH navigation team and other PA-NH staff. In collaboration with the team, the patient navigator coordinates medical care and services, and removes barriers to care, for low income, uninsured and underinsured patients. The patient navigator provides culturally, and linguistically appropriate services aimed at facilitating access to needed medical care in a timely manner.
