Nurse Practitioner or Physician Assistant – Adult Primary Care
Location: Hartford, CT area
Must have primary care experience
Salary Range: $104,312 – $167,772.80 (commensurate with experience)
Clinic Hours:
Monday – Thursday: 8:00 AM – 5:30 PM
Friday: 9:30 AM – 5:00 PM
Closed for lunch daily: 12:00 PM – 1:00 PM
A mission-driven, community-based outpatient clinic is seeking a Nurse Practitioner (APRN) or Physician Assistant (PA) to join their Adult Medicine team. This is a primary care role with a strong focus on preventive health, chronic disease management, and wellness for a diverse adult patient population.

Key Responsibilities:
Provide comprehensive adult primary care services, including diagnosis, treatment, and follow-up
Collaborate closely with physicians and interdisciplinary team members
Refer patients to specialists and coordinate care as needed
Maintain timely and accurate documentation using the EHR
Participate in quality improvement and compliance initiatives
Deliver culturally competent care in a diverse, urban setting
Perform other clinical duties as assigned
Qualifications:
Graduate of an accredited Nurse Practitioner or Physician Assistant program
Licensed or license-eligible in the State of Connecticut
Eligible for DEA and CT controlled substance registration
Strong clinical assessment, diagnostic, and communication skills
Comfortable working in a fast-paced outpatient setting
Experience in urgent care or ambulatory care (1–2 years preferred, not required)
Familiarity with electronic medical records and standard documentation practices
