Healthcare Case Manager
Location: Queens, NYC ONSITE
FLSA: Non-Exempt
Position Summary:
The Healthcare Case Manager ensures quality, cost-effective care for patients by providing case management services to children, families, and adults. This role coordinates care with patients’ support systems and a multidisciplinary care team to ensure services meet individual patient needs.
Responsibilities:
Client Services and Direct Care
Conduct intake, comprehensive assessments/reassessments, and develop individualized care plans.
Coordinate with a multidisciplinary care team and patient support networks to monitor progress and promote positive health outcomes.
Build trusting relationships with patients and families and manage emotional distress during crisis situations.
Provide clinic-based health education alongside regular care visits.
Obtain prior authorization for pharmacy or medical services as needed.
Represent services to recruit patients from underserved populations.
Collaboration and Services
Work collaboratively with patients, families, and external service providers to ensure proper care and follow-up.
Identify and link patients to additional services (e.g., specialty care, entitlements, mental health, substance use treatment, community follow-up, assistance programs).
Respond promptly to inquiries via phone, email, or referrals.
Participate in continuous quality improvement (CQI) initiatives and internal/external trainings.
Communicate about missed or rescheduled appointments and any special assistance needs (e.g., interpreter, transportation) to ensure continuity of care.
Compliance
Maintain accurate, organized, and timely patient records and reports.
Ensure all documentation meets regulatory and internal requirements.
Adhere to policies and procedures, including local, state, and federal requirements.
Qualifications
Bachelor’s degree in health, nursing, mental health, child and family studies, public health, or related human services field with 2 years of relevant experience (or bachelor’s in non-related field with 4 years relevant experience).
Experience in a primary care model with target populations preferred (children/families, serious mental illness, developmental disabilities, substance use disorders, LGBTQ+, HIV/AIDS, chronic hepatitis, immigrants, and/or underserved communities).
UAS-NY certification required within 1 month of hire.
Fluency in Spanish or an Asian language preferred.
Excellent written and verbal communication skills in English.
Ability to work collaboratively and independently.
Proficiency in software applications (e.g., Outlook, Word, Excel, PowerPoint, EMR systems).
Strong customer service skills and ability to resolve patient/provider concerns.
Flexibility to work some evenings and weekends.
Ability to multitask and manage multiple projects in a high-stress environment.
