JOB SUMMARY
The Infection Prevention/Staff Development/MDS Coordinator position is responsible for working with the Executive Director to identify and resolve facility deficiencies with implemented correction plans. They may also identify and address any quality assurance auditing issues. This position also aids in coordinating nursing services programs, providing regulatory education, may review staff performance, and ensures state and federal standards of quality care are adhered to. The IP/SDC/MDSC may also monitor nursing cost center budgets and ensure the department is functioning within federal and state guidelines. This position is responsible for assuring the timely completion of accurate assessments and interdisciplinary care plans for elders that meet Federal and State guidelines. Included are identifying resident acuity and needs, helping to determine specific care needs and communicating needs/expectations to families and responsible parties. The MDS Coordinator with also serve as the facility’s Infection Preventionist and assist the Executive Director with various tasks as outlined below.
ESSENTIAL JOB FUNCTIONS
Infection Prevention/Staff Development Coordinator Responsibilities
- May be asked to conducts assessments of prospective Elders to assure that staff can provide the care needed.
- Recommends and collaborates with the DON regarding in-service education programs to promote nursing staff development for geriatric nursing care.
- Assists in monitoring the job performance of the nursing staff
- Assists the DON in monitoring the staffing levels of each of the cottages; and as necessary, directs staff rotations to meet increased or decreased nursing service demands.
- Assures that there is compliance with the regulations regarding all nursing services.
- Collaborates with the IDT, Pharmacy and the dietician to enhance the quality of care for the Elders.
- Attends required meetings, including, but not limited to, QAPI, Safety, Infection Prevention, Significant Change, Business Development, Clinical Nurse’s meeting, and IDT.
- Assists in preparation of the federally mandated Facility Assessment.
- Stays abreast of changes in regulations and nursing practices, and changes to long-term care environment.
- Responsible for overseeing and implementing Infection Control Program, IP Policies and Procedures, and assures that committee recommendations are carried out.
- Daily purposeful rounding to monitor clinical cares and individual status of the Elders.
- Monitors Quality Improvements and disseminates Quality Improvement initiatives to the nursing staff.
- Upholds the Green House Project values of real home, meaningful lives and empowered staff while simultaneously ensuring compliance with State and Federal regulations.
- Works cooperatively and communicates effectively with the administrative team and family members to ensure ongoing patient-centered care.
- The Infection Preventionist is responsible for mandatory reporting to NHSN, staff education, and ensuring all IP policies and procedures are being followed throughout the facility.
- The Staff Development Coordinator ensures that all staff are trained, competent, and compliant with Federal, state, and facility requirements to safely care.

MDS Coordinator Responsibilities
- Completes assessments, Minimum Data Set (MDS) and care plans for all Elders.
- Monitors completion of MDSs by other disciplines within timeframes prescribed by regulatory guidelines.
- Advises interdisciplinary team members of incomplete or untimely assessments by disciplines other than nursing.
- Ensures accurate, timely completion of the MDS/RAPs/Triggers sheet for Elders
- Initiates, monitors and updates care plans and supporting activities that will result in best possible outcome for each elder.
- Generates and distributes monthly care plan calendar for the following month.
- Attends care plan conferences for each elder and the interdisciplinary team (IDT).
- Maintains and updates all care plans and assessments on a quarterly and as needed basis based on change in conditions and other pertinent issues.
- Reviews all elder incidents and accidents and the 24-hour report daily to ensure that care plans for assigned residents reflect current and changing needs.
- Monitors and ensures completion of 7-day look-back documentation by nursing staff within prescribed MDS timeframe.
- Participates in facility management meetings as requested.
- Monitors and audits clinical record documentation on assigned residents to ensure accuracy and timeliness.
- Advises DON of persistent issues related to non-compliant documentation..
- Promotes and participates in ongoing education, inside and outside the facility, to maintain and enhance knowledge of current / new regulations.
- Maintains current knowledge of RAI Manual and MDS assessment process.
- Attends in-service training and other educational programs as directed or authorized.
- Performs all job duties to ensure that confidential information and Elder rights are always protected.
- Reports unsafe conditions and accidents to supervisor or another designated on-duty manager.
- Supports and promotes facility and company standards for superior customer service by exhibiting positive, courteous, and helpful behavior when dealing with all internal and external customers.
- Complies with established standards described in facility policies and procedures, code of conduct, corporate compliance plan, employee handbook and other company documents and publications
- Stays abreast of changes in regulations and nursing practices, and changes to long-term care environment.
- Regularly visits each Green House Cottage to monitor clinical cares and individual status of the Elders.
- Upholds the Green House Project values of real home, meaningful lives and empowered staff while simultaneously ensuring compliance with state and federal regulations.
- Works cooperatively and communicates effectively with the administrative team and family members to ensure ongoing patient-centered care.

POSITION QUALIFICATIONS
Education, Experience & License:
- Current unrestricted Wyoming Registered Nurse License.
- Associates degree in Nursing required, bachelor’s degree in nursing, preferred.
- Minimum two years’ experience in a supervisory or management capacity.
- Minimum of one year of skilled nursing experience is required.
- Experience with MDS 3.0 preferred.
- Experience with Infection Control.
- BLS, required.
- Central registry, required.
