Nurse Practitioner – Skilled Nursing Facility (SNF)
Full-Time | Fall River, MA

We are seeking a full-time Nurse Practitioner to join a collaborative nursing home program providing care to patients in skilled nursing facilities (SNF) in the Fall River area. This role offers exceptional flexibility, autonomy in practice, and a supportive, team-based environment.

Position Overview
The Nurse Practitioner will provide comprehensive care to SNF patients, working alongside other NPs and each facility’s medical director. Responsibilities include making clinical rounds, developing management plans, and directing patient care. This position offers the opportunity to practice independently while being supported by a strong, collaborative clinical team.

This is a full-time position, 30–40 hours per week, working either four or five days per week, with highly flexible scheduling options.

Key Responsibilities

  • Provide direct patient care in skilled nursing facilities

  • Make clinical rounds and develop individualized management plans

  • Direct and coordinate patient care alongside other NPs and facility medical directors

  • Collaborate with nurses, therapists, pharmacists, administrators, case managers, and physician colleagues

  • Engage patients and families in a compassionate, healing-focused environment

Schedule & Flexibility
This role offers exceptional scheduling flexibility and individualized options, making it ideal for nurse practitioners balancing work with family responsibilities.

Qualifications

  • Licensed Nurse Practitioner in the Commonwealth of Massachusetts

  • Valid DEA certificate

  • Massachusetts controlled substances registration

  • Demonstrated maturity, organizational skills, independence, flexibility, and strong problem-solving abilities

  • Ability to work effectively within interdisciplinary teams

  • Compassionate, patient-centered approach

  • Fluency in Portuguese is a plus, but not required
