Overview: Presbyterian is seeking a  Nurse Referral Coordinator to provide direct patient care to an assigned group of patients in an Outpatient Clinic . Common responsibilities include facilitating and following the cardiac patient from point of referral and then coordinating arrangements for referral care, procedural care and / or long-term cardiac care.
