JOB PUR P OSE:  Conducts re-entry intakes, coordinates release planning, collaborates with Re-Entry Case Managers, and ensures continuity of care for individuals receiving Medication Assisted Treatment (MAT). Serves as the primary non-clinical liaison between Re-Entry, the MAT RN/Medical Staff, case managers, and community-based treatment providers. Ensures individuals have trauma-informed, person-centered re-entry plans addressing MAT continuation and broader re-entry needs including housing, treatment, benefits, transportation, and recovery supports.   The position is fulltime, with a full benefits package.
