The Project Access-New Haven (PA-NH) patient navigator reports to the RN navigation manager and works closely with PA-NH navigation team and other PA-NH staff. In collaboration with the team, the patient navigator coordinates medical care and services, and removes barriers to care, for low income, uninsured and underinsured patients.
