We are a physician-led telemedicine group specializing in post-discharge care to help bridge the gap from hospital to home. Our hybrid care model combines telehealth and in-person care services, which allows our collaborative, multispecialty group to continue to forge solid connections, provide accessible, affordable care, and elevate the healthcare experiences of seniors in post-acute settings. Our team conducts remote telehealth visits with senior patients in their homes, assisted living communities, or skilled nursing facilities with 72 hours of hospital discharge.
