The Healthcare Practitioner is responsible for providing comprehensive, evidence-based primary care services within Lighthouse Community Health Services, an integrated community health program serving medically under-served populations. This role includes evaluation, diagnosis, and management of acute and chronic conditions, health promotion, and disease prevention.
