Overview: The Care Management Patient Navigator provides support for care coordination services to both moderate and high-risk patients with acute and chronic illnesses in a primary care setting. This position will optimize health and management of chronic conditions to prevent/minimize long term complications to patients, assist patients with community resources, medication cost assistance, social determents of health, and coordination of patient appointments.
