Overview: The Care Management Patient Navigator Educator is responsible for providing support for care coordination services to both moderate and high-risk patients with acute and chronic illnesses in a primary care setting through education and collaboration with the healthcare team. As an Educator implements and evaluates orientation needs, acts as a resource change agent, professional role model, mentoring new staff, assists and constantly evaluates patient navigator practice.
