Position Summary The LPN Case Manager supports safe and efficient patient transitions from hospital to home by combining clinical nursing skills with coordinated discharge planning. This role arranges home health services, home IV antibiotics, durable medical equipment, and transportation, while also addressing key social determinants of health such as housing, food access, and medication affordability. Working closely with RNs, social workers, providers, and community partners, the LPN Case Manager ensures patients receive the resources and support needed for optimal health. Essential Duties Coordinate home health services, including outpatient services, nursing, PT/OT/SLP, and supportive care.
