Care Navigator-RN-Bryan Palliative Care GENERAL SUMMARY: Supports and promotes the continuum of care for their patient population. Provides individualized assistance to patients, families and caregivers to promote optimal outcomes. Works collaboratively with physicians, staff and other providers across the care continuum.
