Social Worker - Behavioral Health Professional JOB SUMMARY AND SPECIFICATIONS JOB SUMMARY Provides behavioral healthcare services to patients using a combination of individual, family, group and community work skills. Conducts services to include, but not limited to, assessment, diagnosis, brief interventions, referrals, case management, consultation and staff education. Works as part of a multi-disciplinary team along with physician, nursing staff, case management, and other health care professionals and with patients and families in developing and implementing an appropriate plan of care.
