Overview:
Provides social work services to patients and families within hospital setting in order to meet identified psychosocial needs, as well as to enhance problem-solving and coping capacities and to facilitate timely discharge by arranging post-discharge services. The social worker provides psychosocial assessments, supportive counseling, crisis intervention, community resource connection, financial resources and works collaboratively with all other members of the healthcare team, ensuring clear communication and care planning to all patients.
Responsibilities:
Works collaboratively with the RN Case Manager to provide discharge planning based on assessment of patient and family needs, preferences and available resources in order to ensure timely discharge and provide appropriate linkage to post-discharge care providers (IE New/resumptions SNF, LTAC, Rehab, Hospice, and other Community resources)
