About the role
- The Social Care Manager (Care Manager) provides direct, hands-on support to individuals by delivering care management and 1115 Waiver services. This role focuses on helping clients navigate healthcare and social service systems, addressing social determinants of health, and connecting individuals to critical resources such as housing, food, and medical care.
- You will work closely with clients in the community and remotely, ensuring they receive consistent, person-centered support that improves stability, health outcomes, and overall well-being.
- You will be part of a growing, mission-driven team focused on expanding access to services across New York State.
What you'll do
- Provide direct care management services to assigned clients
- Complete Medicaid screenings, assessments, and re-assessments
- Deliver 1115 Waiver services, including:
- Care Management
- Navigation (medical, behavioral health, community resources)
- Housing support (pre-tenancy, tenancy sustaining, rent/temporary housing coordination)
- Follow-ups and ongoing client engagement
- Develop and maintain individualized care plans based on client needs
- Conduct in-person and remote visits with clients
- Document all services accurately in real time (who, what, when, where)
- Maintain consistent daily billing and unit tracking (15-minute increments)
- Coordinate with healthcare providers, DSS, shelters, and community partners
- Assist clients with applications (housing, benefits, SNAP, etc.)
- Ensure all services are compliant with Medicaid and 1115 Waiver guidelines
- Meet productivity expectations (daily/weekly unit goals)
Qualifications
- Experience in case management, social services, behavioral health, or community outreach
- Knowledge of Medicaid, care coordination, or social care services (1115 Waiver experience is a plus)
- Strong ability to engage clients and build trust
- Comfortable working both in the field and remotely
- Strong documentation and organizational skills
- Ability to manage a caseload and meet productivity expectations
- Reliable transportation (preferred for community visits)
- High school diploma required; Associate’s or Bachelor’s degree in human services, psychology, or related field preferred
