Geriatric Nurse Practitioner – Home-Based Primary Care

Overview:
We are a small, independently owned solo nurse practitioner practice providing comprehensive primary care to older adults in their homes. As our patient panel continues to grow, we are looking to add an experienced and compassionate Geriatric Nurse Practitioner to join our practice. This is an excellent opportunity to step into a low-volume, relationship-focused home-based care model with autonomy, flexibility, and strong support.
Responsibilities:
* Provide primary care visits in patients’ homes, including assessment, diagnosis, treatment planning, and ongoing management
* Manage chronic medical conditions such as heart failure, COPD, diabetes, dementia, and frailty syndromes
* Deliver high-quality, empathetic care during advanced illness and end-of-life stages
* Practice independently in the field, with the lead NP available for collaboration and support
* Order and interpret labs, imaging, and diagnostics
* Coordinate care with specialists, home health agencies, and community resources
* Maintain timely and accurate clinical documentation
* Communicate clearly with families and caregivers regarding treatment plans and goals of care
* Work with a remote medical assistant who provides follow-up support, inbox management, scheduling, and medication refill assistance
Qualifications:
* Board-certified Nurse Practitioner (AGNP, FNP, or similar)
* Experience in geriatrics, primary care, palliative care, or home-based care strongly preferred
* Comfortable practicing independently with remote support
* Strong clinical judgment and excellent communication skills
* Ability to travel locally for home visits
* Compassionate, patient-centered approach to older adult care
