We are currently looking for a compassionate, organized, and team-oriented Home Health Physical Therapist (PT) - Level 2 to serve patients throughout the San Mateo Peninsula. We're currently seeking an Intake & Scheduling Coordinator to join our dynamic San Francisco office. If you're a natural communicator, highly organized, and thrive in a fast-paced healthcare environment- we'd love to meet you!
What You'll Do:
In this vital role, you'll help patients begin their care journey and stay on track with scheduled visits. Acting as the liason between hospitals, providers, patients and our clinical team, you'll ensure an efficient, compassionate, and well-coordinated intake and scheduling process. You'll also support relationship-building with community partners.
Key Responsibilities:
• Coordinate incoming referrals from hospitals, facilities, and providers
• Accurately collect and enter patient information
• Communicate with patients and families about services, scheduling, and care start dates
• Work closely with the clinical team to assign and schedule visits
• Partner with our community liaisons to support timely admissions and promote strong partnerships
• Troubleshoot scheduling challenges and ensure continuity of care

Preferred Qualifications:

Location: In-person at our San Francisco office.

Responsibilities
Provide physical therapy services within the scope of practice, following individualized plans of care
Deliver treatment, evaluate patient response, and report changes to the Physician, Case Manager, and/or Director of Nursing
Maintain appropriate clinical records and submit documentation on company devices per organizational policy
Follow all company policies and procedures
Perform other duties as assigned
Requirements
Current California PT License (required)
Valid California Driver's License
Experience: Minimum 2 years of Home Health experience required. Must be proficient in OASIS and case management.
Coverage Area: Flexible Territory. Assigned a primary territory but must be willing to travel to adjacent zones (e.g., SF + Daly City/Brisbane) when caseload demands.
Ability to travel within San Francisco and surrounding areas
Caring, compassionate personality with strong communication skills
Job Type: Full-Time / Per Diem
Schedule: Flexible. Monday – Friday standard, with rotational weekend coverage (1 weekend per month/quarter) required.
Annual Full-Time Expectation: $150,000 – $170,000 per year
Pay: $72.00 – $81.00 per hour
