*Job Description:*
* Follows the eight rights of medication and treatment distribution and follows JLNATC Nursing Policy and Procedure for preparation, administration, and documentation of same.
* Ensures that patients scheduled for discharge have prescriptions for prescribed medications in place at the time of discharge. Nursing Discharge Note completed in AVATAR and recorded within 24 hours of patient's discharge.
* Accurate counting with oncoming, off-going medication staff of controlled drugs. Medication keys will be signed for and will be always attached to person and are never relinquished to anyone not credentialed and privileged to give medication or without a count between persons.
* In emergency situations, reports signs and symptoms to physician and initiates immediate medical attention. In absence of physician, takes immediate lifesaving measures as appropriate.
* Observes, assesses, and records all client complaints of illness, emotional needs, and will take appropriate action. Notifies physician or appropriate team leader. Takes baseline vital signs when necessary and ensures that follow up with physician occurs as needed.
* Closely monitors those patients that are determined by M.D. to require medical monitoring. Takes vital signs and utilizes specific clinical withdrawal assessment tool as needed. Follows individual protocol per MD orders. Notifies the MD for withdrawal symptoms that are not responding to prescribed medication.
* Assess and provide patients with information and/or documentation containing information regarding their medications, including but not limited to how the medication works, associated risks, intended benefits, side effects, contraindications, the importance of taking the medication as prescribed, potential reactions when combining the medication with non-prescribed medication, alcohol, tobacco, caffeine and illicit drugs, and any other pertinent information.
* Assesses effects of medication and patient knowledge of prescription medications and document in EMARs and Avatar progress note. Documents all nursing interventions in the electronic medical record.
* Follow-up on all staff, patient or building problems. Receives verbal report from Night Charge. Assures that nursing shift reports are adequate and that information regarding all patients and building activities is communicated adequately. The same process is followed with the Evening Charge.
* Reports daily to RN Supervisor or designee any outstanding building or patient problems/concerns.
* Monitors and assures that all staff assignments are delegated to staff in accordance with NYS Civil Service qualifications and standards. Ensures that lunch breaks are scheduled
* Assures adequate dining room coverage for meals during assigned shift.
* Assures that Admission Notes are done adequately on all new admissions.
* Answers ward telephone and distributes messages as needed.
* Assures monitoring of special diets.
* Ensures that visitors or other persons do not have access to the facility without planning or permission. Challenge unknown persons in the facility to ensure security and appropriateness of the environment.
* Any changes in the staff schedule related to call-ins will be reflected on the Inter-shift Log. Contacts will be made as needed to arrange for minimum staffing for the on-coming shift is met by following protocol. Notify RN Supervisor.
