Job Summary:
NHS Management is seeking a highly skilled and experienced Registered Nurse (RN) to serve as a Regional Resident Assessment Nurse. This key role will provide clinical oversight and support to multiple skilled nursing facilities within a designated region, focusing on accurate and timely resident assessments, care planning, and regulatory compliance. The Regional Resident Assessment Nurse will play a vital role in ensuring the delivery of quality care and positive resident outcomes.
Responsibilities:
Provide expert guidance and support to facility staff regarding resident assessment processes, including Minimum Data Set (MDS) completion, care planning, and care delivery.
Conduct regular on-site visits to assigned facilities to review resident assessments, care plans, and clinical documentation for accuracy, completeness, and compliance with federal and state regulations.
Educate and train facility staff on current MDS guidelines, coding practices, and regulatory updates.
Monitor key performance indicators (KPIs) related to resident assessments, such as MDS accuracy, case mix index (CMI), and survey outcomes.
Collaborate with facility leadership, including Directors of Nursing, MDS Coordinators, and Administrators, to develop and implement strategies for improving resident assessment processes and care quality.
Participate in facility surveys and provide support during the survey process.
Assist with the development and implementation of policies and procedures related to resident assessments and care planning.
Stay up-to-date on changes in MDS regulations, coding guidelines, and best practices.
Travel within the assigned region is required.
Qualifications:
Current and valid Registered Nurse (RN) license in the state(s) where our facilities are located (Multi-state license preferred).
Minimum of [Number] years of experience as an RN in a skilled nursing facility setting.
Extensive knowledge of the Minimum Data Set (MDS) 3.0, care planning process, and related regulations.
Strong understanding of Medicare and Medicaid reimbursement systems.
Experience with electronic health record (EHR) systems and MDS software.
Excellent clinical assessment, critical thinking, and problem-solving skills.
Strong communication, interpersonal, and training skills.
Ability to work independently and as part of a team.
RAC-CT certification preferred.
