Title: Clinical Care Nurse Coordinator for OBOT

Reports to: Community Support Services Nurse Manager
Pay: $50-55 (can go higher if need be but ideally up to $55)
Assignment: Until the end of Oct (potential for TTP for right candidate).
Schedule: Monday-Friday 8:30 or 9am to 5 or 5:30pm for 40 hours
Location: 3 days at 34 Haverhill St. Lawrence (Mon, Tues, & Wed) and 2 days at 150 Park St, Lawrence (Thurs & Fri).

Position Summary:
The clinical care nurse coordinator for Office Based Opioid Treatment (OBOT) serves as an integral member of the healthcare team to improve the clinical and operational performance for a subset of assigned patients. The role will focus on organizing, planning and coordinating the delivery of care as provided by the healthcare team at the direction of the PCP. She/he will manage and operationalize the integration of care in office or remotely dependent upon program and patient needs in order to promote and improve health outcomes for the subset of OBOT patients; patients who often have a substance abuse disease. She/he will focus on making the right care available to the patient at the right time. The incumbent will be proficient in understanding NCQA, JCA, and Meaningful Use requirements and will be knowledgeable in systems improvement concepts and usage.
