Title: Clinical Care Nurse Coordinator for OBOT

FLSA: Non-Exempt
Reports to: Community Support Services Nurse Manager
Pay: $50-55 (can go higher if need be but ideally up to $55)
Assignment: Until the end of Oct (potential for TTP for right candidate).
Schedule: Monday-Friday 8:30 or 9am to 5 or 5:30pm for 40 hours
Location: 3 days at 34 Haverhill St. Lawrence (Mon, Tues, & Wed) and 2 days at 150 Park St, Lawrence (Thurs & Fri).

Position Summary:
The clinical care nurse coordinator for Office Based Opioid Treatment (OBOT) serves as an integral member of the healthcare team to improve the clinical and operational performance for a subset of assigned patients. The role will focus on organizing, planning and coordinating the delivery of care as provided by the healthcare team at the direction of the PCP.  She/he will manage and operationalize the integration of care in office or remotely dependent upon program and patient needs in order to promote and improve health outcomes for the subset of OBOT patients; patients who often have a substance abuse disease. She/he will focus on making the right care available to the patient at the right time. The incumbent will be proficient in understanding NCQA, JCA, and Meaningful Use requirements and will be knowledgeable in systems improvement concepts and usage. 

Job Responsibilities and Performance Standards:

• Supports patients through the coordination of care for designated OBOT patients at specific assigned site/s or for specific substance abuse.
• Ensures the development of, and maintains constructive relationships between, OBOT case management staff and with staff from all interfacing departments.
• Works collaboratively with Medical Services, Nursing Department, Operations and Information Technology and other departments’ leadership to implement all substance abuse elements.
• Utilizes ascribed processes for managing the needs of complex patients, initiating interventions based on physician approved patient-specific protocols and order sets (i.e. suboxone and other substance abuse medications).
• Communicates with internal and external care providers to ensure safe and effective care management.
• Contributes to the documentation of individualized care plans for identified patients focusing on specific treatment goals.
•  Assesses progress toward goals based on clinical judgment, review of patients’ self-monitory tools and trends in clinical data.
• Assists with the timely follow-up and coordination of care for patients discharged from a hospital or other healthcare organization in the continuum.
• Provides direct patient care as ordered by clinician, which may include immunizations and directly observed therapy via injectable and or oral medications.
• Serves as a member on care team at assigned site.
• Reviews, analysis and utilizes data and trends from relevant reports to determine if case management has improved patient status.
• Participates in quality improvement activities, assisting in the design and implementation of multifaceted projects.
• Engages in and facilitates other initiatives as assigned that may cross sites or disease process that align with the PCMH model.
• Facilitates a culture of a PCMH. 
• Sustains compliance with all professional and organizational requirements to meet position standards.
• Become knowledgeable about the GLFHC Improvement plan and the model for improvement endorsed by the organization.
• On a regular basis participate in ongoing quality improvement activities related to individual, team, and organizational performance improvement.

Qualifications:
Experience

• Minimum of two years nursing experience with 6 months acute care preferred.
• Prior experience in a Substance Abuse Program is strongly preferred.
• Familiarity with computerized data management and research protocols.
• Sound analytical and computer skills required.
• Strong verbal and written skills required.
• Spanish fluency preferred.

Education

• Graduate of an accredited professional nursing program with current Massachusetts Registered Nurse license
• Bachelor’s degree in nursing or related field required.

Physical Environment

• Physical surroundings are satisfactory with occasional exposure to injury or unpleasant elements such as chemicals or infectious hazards which require precautions.

Social/Psychological Conditions

• Regularly working at clinic full-time days
• Close and consistent communications with staff. Daily interactions with multi-cultural population which is primarily Latino.
• Flexibility with alterations in schedule/changes to accommodate responsibilities.

Physical Effort

• Regular and routine travel between clinic sites required with occasional travel in inclement weather.
• Frequent sitting on regular basis with frequent reading; light physical effort as in standing, walking, bending and occasional lifting of light items up to 25 pounds; occasional pushing of light items up to 40 pounds.
• Manual dexterity, good speaking and hearing abilities.
• Occasional exposure to contagious diseases, hazards and slippery surfaces.
