As the RN Case Manager you will oversee all medical support services for each person living in the Medical Model (24/7 LPN staffed) home and for people requiring nursing intervention/monitoring in non-medical model homes based on RN Assessment and Nursing Care Plan, including training non-licensed staff for anticipated emergencies and delegation of nursing tasks as per OSBN rules, medication management and medical records.
